The breast is a symbol of femininity and is of crucial importance to many women in terms of how they perceive their bodies. Many women want to feel good about their bodies and want to convey this to the outside world. If you feel uncomfortable with the shape of your breasts and find them aesthetically unsatisfactory, this can significantly affect your self-esteem. In such cases, breast augmentation and, in particular, breast lift surgery—also known as mastopexy—are popular solutions in aesthetic surgery.

There are many reasons for sagging breasts. It often occurs as a result of volume loss after pregnancy and breastfeeding or after significant weight loss. Age can also weaken the connective tissue and lead to sagging breasts.

This is how Breast Lift surgery works:

Preliminary meeting

Before the procedure, we first conduct a consultation in our plastic and aesthetic surgery practice, followed by an examination of the breast by our specialist, Dr. Münzberg. The breast is measured to determine the extent of the lift. You also decide whether the breast should have a natural shape or a slightly décolletage shape. This allows us to ensure that you can clearly visualize the result of the breast lift.

In preparation for the surgery, you should refrain from taking blood-thinning medications such as aspirin for about two weeks. You should also refrain from alcohol and nicotine, as these can have a negative effect on wound healing. Please also inform Dr. Münzberg about any medications you are taking and any allergies you may have.

And remember: if you want to have a breast lift after pregnancy, you should have stopped breastfeeding for at least 8 months. Only then will the shape of your breasts have stabilized again.

The aesthetic treatment

Breast lift surgery can be performed using four different techniques:

  • Internal bra technique
  • Lejour technique / I-technique
  • Benelli technique

In order to avoid large scars on sagging breasts, breast lifts are now performed using a technique that minimizes scarring. You will only have one scar around the nipple and one vertical scar extending to approximately the breast fold. This technique is known as the lollipop incision and is a further development of the Lejour technique. The internal bra technique is a relatively new technique. Dr. Münzberg makes two approximately 1 cm wide strips from the dermis (excess deep skin layer) in front of the lower mammary gland tissue to the breast fold. There he crosses them and fixes them to the chest muscle with a suture. The result lasts for many years, even with the aging process and connective tissue weakness. And if desired, we can also reduce the size of the areola and reposition it.

The T-incision, a surgical technique that leaves an additional scar in the breast fold, is rarely used by Mr. Münzberg and only for very large and severely sagging breasts. The scar often has to be extended outwards and inwards, meaning that it may be visible outside of a bra or bikini.

The Benelli technique, on the other hand, is minimally invasive: it only requires an incision around the areola. However, this technique can only be used for relatively minor sagging.

In all surgical methods, the excess skin is removed and then sutured with fine medical suture material. Mr. Münzberg will be happy to show you before and after pictures of breast lifts performed in his practice upon request.

The surgery is usually performed under general anesthesia, followed by an inpatient stay of at least one night. During this phase, our experienced nursing staff will be there for you. Aftercare

You must wear a support and compression bra continuously for at least 3 months after the operation. It is essential that you take it easy for the first few days after the treatment. You should also avoid full baths, saunas, and sporting activities that could impair wound healing for at least 4 weeks. Sun exposure and solariums are detrimental to scar healing. You will be given a support bra after breast surgery to provide good support for your breasts. After the recovery period, we recommend that you wear a suitable sports bra during sporting activities. After six to eight weeks, the scars will have healed and the results will be fully visible. Follow-up appointments will continue to be scheduled 3 and 6 months after surgery.

Combination with breast augmentation or breast reduction

It is possible to combine a lift with an additional change in breast volume, and this can be done in a single operation. If breast augmentation is to be performed at the same time as the breast lift, an implant is also placed under the chest muscle. Dr. Münzberg will examine you again and explain the shape, type, and size of the implant. During the additional breast reduction, excess tissue is removed. Dr. Münzberg will explain how much tissue needs to be removed during a personal consultation at his practice. Breast lift surgery does not affect the sensitivity of the nipples. If you wish to breastfeed in the future, this is possible as the milk ducts are not located in the area of the operation and are therefore not severed. A breast lift can be performed on an outpatient or inpatient basis under general anesthesia.

Depending on the findings and further treatment such as breast augmentation or reduction, the operation takes approximately 1 to 3 hours.

Costs of a Breast Lift

The cost of a breast lift is not fixed. The extent of the surgery required, the type of anesthesia, and whether an inpatient stay is necessary determine the cost.

In addition, a breast lift can be combined with other procedures such as breast reduction. In some cases, health insurance will cover part of the costs. Women with overly large breasts often suffer from back and neck pain and recurrent inflammation in the breast fold. Cosmetic plastic surgery can significantly improve quality of life and also address health issues. Have yourself examined by a doctor so that you can provide medical evidence of the necessity of the procedure. You can then ask your health insurance company to cover all or part of the costs.

Additional breast augmentation is a purely cosmetic procedure, so it is unlikely that your health insurance company will cover all or part of the costs.

Breast implants are not necessary for a breast lift alone! However, if you also wish to increase your breast size, an implant must be inserted. These implants are placed under the pectoral muscle.

Dr. Münzberg, a specialist in this field, recommends silicone gel breast implants. For breast lifts, we exclusively use high-quality, safe breast implants from the German company Polytech (polytech-health-aesthetics.com) that are CE-certified and bear the EU seal of approval. These breast implants are available in all desired sizes and shapes.

The breast implants used in a breast lift have six layers, including a leak-proof barrier layer, and are filled with a cohesive silicone gel. The new generation of implants features a highly cross-linked cohesive silicone gel that always returns to its original shape even under extreme stress (memory effect). In the past, various implants (soybean oil implants, hydrogel implants, PIP, etc.) had to be withdrawn from the market for various reasons due to their fillings and shell structure (titanium coating).

Replacing breast implants every 10 years, as was previously recommended, is no longer necessary today. Generally, the prevailing opinion today is that breast implants can remain in the body for a lifetime, provided no complications arise. Implant manufacturers offer a lifetime warranty on the implants. The warranty covers both a free replacement of the implants and the surgical costs. After the surgery, you will receive an implant ID card and a warranty card from the manufacturer.

Another important factor is the filling of the implants: the implants should be filled to 80% or 100% to prevent wrinkling and ensure a good shape in any position. The cohesive silicone gel should ideally feel natural.

To further protect patients, Polytech has product liability insurance for breast implants. In the event of complications with the implants, all costs for a repeat surgery and the implants themselves are covered by the insurance.

The reasons for a breast lift (possibly combined with augmentation) are always aesthetic in nature. Often, the underlying cause is either weight loss or a loss of volume following breastfeeding. In some cases, sagging is also genetically determined and may prompt a desire for a lift at Dr. Münzberg’s specialist clinic.

Prior to a breast lift, a detailed consultation takes place at Dr. Münzberg’s specialist clinic. During this consultation, the surgical techniques and the steps involved in the procedure are explained to the patient. Furthermore, the desired future appearance of the breasts is discussed in detail. For this reason, a so-called “fitting” appointment is also scheduled, during which the size of the implants, as well as the shape of the breasts, are discussed in particular. Potential risks are also an important aspect of the consultation. Additionally, immediately before the surgery, the incision lines are marked and photographic documentation is taken. Furthermore, the shape and size of the breast, as well as a possible reduction of the areola, are discussed. Preparations for the surgery include a blood test, an ECG, the surgical consent form, and a consultation with the anesthesiologist.

Both breast lifts and breast augmentation with implants are now performed using a scar-minimizing technique. In the past, the so-called T-incision was used, which left a scar in the inframammary fold and was often visible even outside the bra or bikini line. Dr. Münzberg performs breast lifts using the scar-minimizing Lejour technique when the breasts are not too large; in such cases, a T-incision is unavoidable.

With the scar-minimizing technique, there is a barely visible scar around the areola and a vertical scar extending toward the inframammary fold. The additional scar that used to result from the so-called T-incision is eliminated. With this new surgical technique, it is important to note that the final result of the procedure will not be visible until 3 to 6 months later. A support and compression bra should be worn for 3 months postoperatively.

Using a new technique for breast lift surgery, Dr. Münzberg also creates an internal “bra,” ensuring that the results last for many years despite the aging process and weakening of connective tissue. In this new breast lift technique, two strips approximately 1 cm wide are dissected from the dermis (the excess deep layer of the skin); these strips extend in a crossed pattern in front of the lower breast glandular tissue up to the inframammary fold and are secured to the pectoral muscle with a suture.

With this surgical technique, the sensitivity of the nipples is preserved. There are no obstacles to a future desire to have children or to breastfeeding, as the milk ducts are not located in the surgical area. The procedure can be performed on an outpatient basis or as an inpatient under general anesthesia.

Perimammary Breast Lift
In cases of only slight or moderate skin laxity, a breast lift can be performed solely around the nipple (a “tobacco pouch”-like lift). In this procedure, excess skin around the nipple is removed. However, breast tissue is not removed during this surgical procedure.

Nipple Lift
With this method of breast lift, the nipple can be raised by approximately 1 to 1.5 cm. It is important to note that the shape of the nipple does not change. This involves a crescent-shaped excision of the breast skin above the areola. However, if the entire breast is sagging and the nipple has dropped as well, a breast lift is necessary.

After the breast lift, two elastic straps are placed above and below the breast (known as “Stuttgart straps”), which must be worn for 5 days. Afterward, a support and compression bra must be worn for 12 weeks. The stitches around the nipple are removed after 10 days, and the vertical sutures after 16 days. Showering is permitted again after that. You should wait approximately 6 weeks before resuming exercise.

The follow-up appointments at the clinic after surgery are as follows: the 1st post-op day for dressing changes and removal of the drains; the 5th day for dressing changes; the 10th day for partial removal of the sutures; the 16th day for removal of the remaining sutures; and follow-up visits at 3 weeks, 6 weeks, 3 months, and 6 months.

Every surgical procedure carries risks. In the case of a breast lift, these may include: capsular contracture (if implants were also inserted), impaired wound healing, bruising, prolonged swelling, pain, redness, and discoloration from hematomas, wide, raised, and visible scarring, and asymmetry. Melting of fat/glandular tissue, loss of sensation in the nipples, circulatory disorders leading to necrosis of the nipples, and risk of thrombosis/embolism.

With the amendments to the German Law on the Advertising of Medicinal Products that took effect in October 2012, the ban on comparative “before-and-after” images was somewhat “relaxed” (→ see the ruling of the Higher Regional Court of Celle; Case No. 13 U 160/12). Dr. Münzberg’s clinic also complies with the German Law on the Advertising of Medicinal Products when it comes to stating the prices of surgical procedures, as this law prohibits misleading advertising. Under the law, it could appear as though the outcome of a surgical procedure is for sale and that success is guaranteed. For this reason, the website does not list the costs of a breast lift.
